Responsibilities:

A clinically competent, licensed practical nurse who provides care to a group of patients/residents under general supervision/delegation of a registered nurse according to established policies, procedures, protocols and standards of practice and care. Effectively interacts with patients/residents, families, responsible party, and all health team members.

Qualifications Required:

  • Current state licensure as a Practical Nurse, which must be in good standing
  • CPR certified
  • Must have good communication skills
  • Must be able to read, write and communicate in English

Qualifications Preferred:

  • Clinical experience preferred

Responsibilities:

The Certified Nursing Assistant (CNA) will provide patient/resident care as required under the direct supervision of the licensed nurse. The CNA completes assigned tasks to ensure timely, efficient and safe patient/resident care. Communication and interactions with patients, residents, families and other health team members is an essential component of this position. The CNA provides routine daily nursing care using a philosophy of Person Centered Care.

Qualifications Required

  • Must be in good standing on the State Nursing Assistant registry or appropriate entity
  • Must have good communication skills
  • Must be able to read, write and communicate in English
  • A high school diploma or equivalent
  • Basic computer skills

Qualifications Preferred:

  • Previous hospital or nursing home experience
  • CPR certified

JOB SUMMARY

The RN is responsible for developing and implementing appropriate plan of care based on the residents’ needs, evaluating and making the necessary changes to the plan of care.Responsibilities: Plans and provides education to other health care team members, residents, family, and caregivers on information regarding the care plan for the residents. Demonstrates the knowledge of medication administration and evaluates their responses to the medications. Plans for residents’ admission/discharge and anticipates for long term and/or short term residents’ needs.

QUALIFICATIONS REQUIRED

  • Current state licensure as a Registered Nurse (RN), in good standing.
  • BLS certified.
  • Good communication skills.
  • Ability to read, write and communicate in English.

QUALIFICATIONS PREFERRED

  • Clinical experience.

Responsibilities:

Portions and serves food attractively and assists in food preparation for residents. Operates, cleans and sanitizes kitchen and cafeteria equipment, dishes, and utensils. Adheres to food safety guidelines, follows diet restrictions and recipes, receives stock and puts it away. Has effective communication and interaction with residents, patients, visitors and other team members.
